Abstract

Background
Histopathologic discordance between intestinal locations in canine chronic enteropathy has prompted recommendations to biopsy both duodenal and ileal sites to minimize lesion oversight. Further evaluation is required for dogs with non-chronic enteropathies.
Objective
To determine the histopathologic concordance between duodenal and ileal endoscopic or full-thickness biopsy specimens for all dogs regardless of underlying diagnosis, and to assess association with clinicopathologic variables.
Animals
One-hundred and twenty-four client-owned dogs that had endoscopic (112) or full-thickness (12) biopsy specimens collected from the duodenum and ileum.
Methods
Retrospective study of electronic medical records for canine cases with concurrent biopsy specimens collected from the duodenum and ileum. Concordance of inflammatory cell type and severity was assessed by a single board-certified veterinary pathologist using the modified World Small Animal Veterinary Association (WSAVA) Gastrointestinal Standardization Group guidelines. Relevant clinicopathologic variables were recorded for all dogs.
Results
Equivalent rates of full concordance (33%) existed between endoscopic and full-thickness biopsy specimens. Lower discordance for cell-type was observed in the endoscopy group (11.6%), although included 4 duodenal neoplasia cases, without concurrent ileal involvement. The prevalence of enteric associated T-cell lymphoma-2 (EATL-2) was 4.8%. Histologic severity had a higher discordance rate. No clinicopathologic variables demonstrated statistically significant association with discordance.
Conclusions and clinical importance
Comparable histopathologic concordance rates exist between endoscopic and full-thickness biopsy specimens. The level of discordance necessitates concurrent biopsy of both duodenal and ileal locations in all suspected enteropathy cases. However, the rate of EATL-2 was lower compared to cats and showed a predilection for the duodenum.
